Skilled Nursing Facilities

Residents in nursing facilities require custodial care, ongoing skilled nursing support and a wide range of medical products. Every month, our consultant pharmacists work onsite in nursing facilities where they review charts for each resident, inspect nursing units, review narcotic documentation (and/ or assist with destruction), conduct med pass observations and develop medication trend analysis/ reports for individuals and the facility. As follow up to the review and analysis, pharmacists make clinical recommendations to physicians and/ or facility nursing staff. Our pharmacists may also participate in facility committees, state pre-surveys, problem-solving situations and system development. The focus of monthly onsite pharmacy support is patient safety, and onsite consultant services comply with or exceed all state and federal regulations.

Assisted Living Facilities

We provide a range of medical products to residents in assisted living facilities, residential facilities where residents require assistance with daily living but do not require skilled nursing services. Every quarter our pharmacists review individual charts, inspect med rooms and units, and review narcotic documentation. The focus of quarterly onsite pharmacy support is patient safety, and quarterly onsite services comply with or exceed all state and federal regulations.

Group Homes/Mental Health

Residents in group homes require different levels of assistance with their daily living, so we offer multiple packaging options to fit each patient’s needs. Our unique strip packaging organizes medications by day and time and includes reminders for medications that will not fit in a packet, such as inhalers and insulin. This packaging – combined with the provided comprehensive medication list – simplifies the medication process for the caregiver and/or patient. We can also provide traditional vial or blister card packaging if this best suits the needs of the home setting.

Independent Living and At-Home Services

Seniors living independently or at home often have multiple medications and complicated dosing regimens. Often, medication non-adherence can result in a loss of independence. We provide medications conveniently packaged and organized by day and time to simplify the daily drug regimen. This unique packaging – combined with the provided comprehensive medication list – eliminates the danger of missing a dose or taking outdated, expired, or discontinued medications from multiple bottles at home. The easy to use packets increase adherence to medication regimens and can help reduce adverse events resulting from poor medication management.

Discharge Services

When a patient is discharged from an acute setting (hospital or rehab facility), their medication list is often extensive with complicated dosing regimens. We provide medications conveniently packaged and organized by day and time. This unique packaging – combined with the comprehensive medication list – eliminates the danger of a newly discharged patient missing a dose or taking outdated, expired, or discontinued medications from multiple bottles at home. The easy to use packets increase adherence to medication regimens and can help reduce readmission to the hospital – especially in the first 30 days after discharge.

Delivery

Regular, scheduled deliveries and emergency deliveries to our communities for urgent needs – no need to coordinate with a retail pharmacy.  Each scheduled delivery includes consolidated delivery sheets with resident name, medications delivered and quantities delivered. Each delivery sheet has a bar code that our delivery staff scans and uploads straight to our pharmacy after facility signature. Our delivery services are an important part of our overall quality of service.

IV Therapy

IV infusion therapy and compounding services meet the special needs of patients requiring additional care. We provide IV therapies and the IV pumps to support them. IV therapies are used most frequently by patients to avoid potentially life-threatening infections or to provide end-of-life pain management. Our customized compounding products provide facility staff with cost-effective and ease-of-medication administration options for a broad range of patient-specific needs.

Medicare Part D Coverage Guidance, Education, and Management

During open enrollment each year, many residents reevaluate their Medicare prescription drug coverage (Part D) plans. Our pharmacy meets with residents and their families annually to educate them about their various options and the coverage gap. We also take an active role and notify the resident when they are near the donut hole so there are no surprises when their bill arrives. To further eliminate billing frustrations, we also proactively identify non-covered medications when prescribed and work with the doctor to determine a covered option in order to keep costs low.

Regular Family Nights

To meet the needs of our residents, we often host family night events at the communities we serve. The purpose of these family nights is to be able to share more information about Guardian’s programs and services. These in-person events give the pharmacy and the facility staff an opportunity to answer questions about a variety of topics, including insurance processes and resident care. Residents and their families are also able to voice any concerns or issues they might have and feel confident that our pharmacy has a solution.

Innovative Technology

Resident-specific supplies of routine medications are delivered electronically to our pharmacy 24/7. This eliminates financial and environmental waste associated with the traditional packaging resulting from therapy changes. Medications are packaged by time and order of administration and help reduce med errors and save time during a med pass.  For discharge patients, independent living, and at-home patients, 30 day supplies of patient specific medications are packaged by day, time, and order of administration to help improve compliance.

eMAR Integration

We can interface with all leading electronic health record applications (EHR) and support the community in transitioning to and utilizing an eMAR or EMHR platform.
